Student Update Form
At the beginning of each year, a Student Update Form will be sent to you to amend any changes to your personal details.
Emergency Contacts
Although we are fortunate to have a part-time nurse, our facilities are for emergencies only and not for full-time care. It is, therefore, imperative that your child’s emergency contact details are up-to-date and that someone can be contactable at all times.
Your Email Address
We require all parents/guardians to provide us with a valid personal email address. We have commenced communicating via email on a regular basis, as this is a very cost-effective method. For those without an email, we will continue to send information home via your child or by mail.
Health Conditions
If your child has a medical condition of which the school should be aware, please indicate on the form. The relevant Student Health Care Forms will then be sent to you.
If you have changed your contact details, please come to the office or download and complete the Change of Contact Details form. This form must be completed and signed by a parent/guardian.
Student Health Care Plan
Health Conditions
It is a requirement of the Department of Education that all health care plans are to be updated annually. If you have previously indicated on your child’s enrolment form that your child has a health condition, we require new Student Health Care Plans to be completed each year. Please contact the office by telephoning 9345 8200 to request this form to be sent to you and return the completed form as a matter of urgency.
